$495 TRANSITION ASSESSMENT & PATHWAY PLAN AGREEMENT
A defined planning engagement, with no obligation to buy more.
This agreement governs the one-time $495 Transition Assessment & Pathway Plan arranged directly with Long Pathways. If the same household later enters a signed Pathways Start, Guided Pathways, Pathways Complete or Pathways Custom engagement, the full $495 planning payment is applied once as a credit against the Long Pathways service fee shown on that engagement's invoice. The electronic signer confirms they are the client or are authorized to arrange the planning service for the identified client. Agreement version 2026-09-12-v7.

Included service
The service includes review of the approved intake, coordinator organization of established facts and open questions, a personalized 30-day action checklist, a coordinator-reviewed 30/60/90-day roadmap, a human-reviewed written Pathway Plan, a plan presentation, and six months of Family Portal access. The checklist is a paid plan deliverable and is not offered as a free generic PDF. The service price is a one-time $495 fee plus any tax required by law. Payment may be completed through the Stripe-hosted checkout linked from the Long Pathways website after the household contact and applicable terms are confirmed.
Not included
The service does not include physical organizing, packing, moving, appraisal, sale, donation, shipping, provider fees, legal documents, fiduciary accounting, tax advice, medical care, or ongoing case management. Any additional service requires a separate written scope and price. The client is not required to purchase more services.
$495 Pathway credit
The Transition Assessment & Pathway Plan is a complete standalone service. If the same household later enters a signed Pathways Start, Guided Pathways, Pathways Complete or Pathways Custom engagement, the full $495 planning payment is applied once as a credit against the Long Pathways service fee shown on that engagement's invoice. The credit does not cover tax, travel, outside-provider charges, reimbursable expenses or a project reserve. It has no cash value, is not transferable, and may not be combined with another planning credit unless Long Pathways confirms otherwise in writing.
Client responsibilities
The client will provide materially accurate information, identify decision makers and known authority limits, attend or designate an appropriate participant for the planning session, protect private delivery links, and promptly identify corrections. Long Pathways may rely on supplied facts while clearly marking unresolved or unverified information.
Authority and decisions
Payment, account access, family relationship, or participation in the planning process does not establish ownership, legal authority, approval, or a disposition decision. The owner or properly authorized representative retains all final decisions. Long Pathways may pause work and request documentation when authority is uncertain.
Scheduling and rescheduling
Scheduling opens after the agreement and separately arranged payment are confirmed. A requested time is not final until confirmed. Please provide at least 24 hours’ notice to reschedule. Long Pathways will make reasonable efforts to accommodate emergencies and may reschedule if the appropriate participants or essential information are unavailable.
Cancellation and refunds
The service may be canceled for a full refund before the earlier of the confirmed planning session or the start of substantive personalized review. After substantive work begins, Long Pathways may deduct the reasonable value of completed work and nonrecoverable costs and refund the balance. No refund is due after the planning session and completed plan are delivered, except where required by law or Long Pathways fails to provide the agreed service. This paragraph does not limit any non-waivable cancellation right provided by applicable law.
Plan delivery and clarification
The completed plan may be released through a private, time-limited link after the engagement is confirmed and the plan receives human quality review. The client should identify factual corrections or questions within seven calendar days after presentation so Long Pathways can clarify or correct the delivered plan within the agreed scope. A dated downloaded or printed plan is a snapshot; later authorized Portal updates are not automatically reflected in an earlier copy.
Professional boundaries
Long Pathways provides practical transition coordination, not legal, tax, financial, medical, appraisal, brokerage, fiduciary, or mental-health advice. Regulated questions should be directed to an appropriately licensed professional. The plan is not a substitute for emergency services or a court order.
Outside providers
Any mover, appraiser, estate-sale company, auctioneer, donation organization, shipper, storage provider or other specialist is an independent business unless a signed agreement expressly states otherwise. Long Pathways may present neutral information and preserve provider-supplied documents. The client selects, contracts with and pays the provider directly. Long Pathways is not a party to that contract and does not become the provider’s employer, agent, joint venturer, guarantor or insurer. The client remains responsible for reviewing the provider’s credentials, insurance, terms, charges, custody and work. Long Pathways remains responsible for duties expressly assumed in this agreement and for conduct that cannot lawfully be excluded or limited.
Virtual administrative assistance
When included in the planning scope, Long Pathways may organize information, prepare questions, facilitate client-attended communications, maintain a transition timeline and record decisions made by the client or authorized representative. Long Pathways may not sign a provider contract, accept an estimate, select a provider, incur a provider charge or bind the client. Administrative support surrounding an interstate move does not include procuring or arranging household-goods transportation.
Privacy and recording
Information is handled under the Privacy Policy and, when applicable, the Consumer Health Data Privacy Notice. A session will not be audio- or video-recorded by Long Pathways without separate notice and consent. The client should not record another participant without all permission required by law.
Electronic acceptance and law
The signer agrees to use electronic records and an electronic signature for this transaction. The recorded signer name, capacity, timestamp, agreement version, document hash, and security-related acceptance record evidence agreement. A printable signed copy is made available after acceptance. Arizona law governs, subject to non-waivable consumer law. Concerns should first be sent to Long Pathways for good-faith resolution; unresolved claims may be brought in the courts serving Maricopa County, Arizona.
