CONSUMER HEALTH DATA PRIVACY NOTICE
Additional care for health-adjacent information.
This notice supplements the Long Pathways Privacy Policy when information handled through our services may be treated as consumer health data under Washington’s My Health My Data Act or a similar law. Long Pathways is a transition-management company, not a health-care provider.

Scope
Consumer health data can be broader than a medical record. Depending on context, it may include information about mobility, accessibility needs, living arrangements, care preferences, a health-related move, accommodations, or an inference about a person’s physical or mental health. This notice applies only where relevant law treats information in our control as consumer health data.
Data we may collect
We may receive accessibility or mobility preferences, requested accommodations, living-transition information, scheduling notes, household documents, messages, or other details a client chooses to provide for coordination. Please do not provide diagnoses, medical charts, insurance identifiers, full Social Security numbers, or other clinical records unless Long Pathways specifically requests them through an approved secure process.
Sources
We collect information from the consumer; a family member, fiduciary, attorney, care professional, or other person acting with appropriate permission; authorized household participants; and service providers supporting the requested transition. We may also create limited operational inferences needed to provide an accommodation or coordinate a requested service.
Purposes
We use the information to provide requested transition planning, support accessibility, coordinate schedules and authorized providers, maintain the household record, protect safety and security, respond to requests, and comply with law. We do not use consumer health data to make eligibility decisions for insurance, credit, employment, housing, or health care.
Sharing
We may share information with processors that host, secure, transmit, or support the service; with a provider or professional the consumer or authorized representative selects; with household participants according to their permissions; or when required by law. We disclose only information reasonably necessary for the approved purpose.
No sale or targeted advertising
Long Pathways does not sell consumer health data and does not use it for cross-context behavioral or targeted advertising. If that practice ever changes, we will provide the notice and obtain the authorization required by applicable law before the change takes effect.
Your rights
Where applicable, you may request confirmation and access, a list of third parties or affiliates with whom data was shared, deletion, or withdrawal of consent. You may also appeal a denied request. Submit a request through the Account center, the Contact page, or by emailing info@longpathways.com with the subject “Consumer Health Data Request.” No account is required to make a request.
Verification and response
We will reasonably verify identity and authority without requiring more information than necessary. We ordinarily acknowledge requests promptly and aim to complete them within 30 days, subject to the time, extension, and exception rules of applicable law. An appeal receives a separate human review.
Deletion and retention
A deletion request may require us to notify processors and other recipients as required by law. Limited records may be retained for security, fraud prevention, legal claims, contractual duties, or another permitted purpose. We will explain any material exception that applies.
