STATE PRIVACY ADDENDUM
Supplemental rights where state law applies.
This addendum supplements the Privacy Policy for residents whose state privacy law applies to Long Pathways LLC and the information at issue. Eligibility, exemptions, definitions and response periods vary by state.

Potentially covered jurisdictions
Depending on applicability thresholds and exemptions, rights may arise under laws in California, Colorado, Connecticut, Delaware, Iowa, Indiana, Kentucky, Maryland, Minnesota, Montana, Nebraska, New Hampshire, New Jersey, Oregon, Rhode Island, Tennessee, Texas, Utah, Virginia, Washington, or another state. This list is informational and does not limit a right granted by applicable law.
Categories and purposes
The categories collected and purposes of use are described in the Privacy Policy and Consumer Health Data Privacy Notice. They may include identifiers, contact and account details, service and transaction information, household content, communications, professional information, limited technical records, accessibility information, and inferences needed to provide the requested transition service.
Rights
Where applicable, you may request confirmation, access, correction, deletion, or a portable copy; obtain information about categories or specific recipients; withdraw consent; limit certain sensitive-data uses; or opt out of sale, targeted advertising, or qualifying profiling. Long Pathways does not currently sell personal information or use it for cross-context behavioral advertising or decisions producing legal or similarly significant effects.
Requests
Submit a request through the signed-in Account center, the Contact page, or by emailing info@longpathways.com with the subject “State Privacy Request.” You may also call 602-980-9680 for assistance. No purchase is required, and no account is required for an email or Contact-page request.
Verification and authorized agents
We will match information reasonably related to the request and may ask for limited additional confirmation. An authorized agent may submit a request where permitted, but we may verify the agent’s authority and, when allowed, confirm the consumer’s identity or direction. We will not ask for account passwords or complete financial credentials.
Timing and appeals
We aim to acknowledge requests promptly and complete them within 30 days, although the controlling law may allow a different response period or extension. If we deny a request, the response will explain the basis and any available appeal process. Appeals may be submitted using the same channels with “Privacy Appeal” in the subject and receive a separate human review.
California disclosures
California residents may request information about collection, use, disclosure, and retention and may exercise other rights available under the California Consumer Privacy Act as amended. Long Pathways does not offer a financial incentive for personal information and does not discriminate for exercising an applicable right. The Website Privacy Policy is intended to satisfy applicable online-privacy disclosure requirements, including California’s notice expectations.
Sensitive and health-adjacent information
Long Pathways uses sensitive information only as reasonably necessary to provide and secure the requested service, support accessibility, comply with law, or as otherwise permitted with consent. Washington and similarly situated consumers should also review the Consumer Health Data Privacy Notice.
Household and professional records
Some information may be controlled by a client, household owner, fiduciary, law firm, or other professional rather than Long Pathways. We will route or coordinate a request when reasonably possible and explain when another party is responsible. A request cannot override another person’s privacy, legal privilege, fiduciary duty, litigation hold, or valid authority restriction.
Non-discrimination and contact
Long Pathways will not deny service, charge a discriminatory price, or retaliate because a person exercised an applicable privacy right. Questions, complaints, requests, and appeals may be directed to Long Pathways LLC using the contact information below.
