Use the app—or work directly with your coordinator.

Clients and family members may use the app when it is helpful. Long Pathways can also gather decisions by phone, video or in person and update the record as permitted by the selected Pathway. A family member may participate digitally without requiring the older adult to manage technology.

You remain in control of outside services.

Under the Transition Assessment & Pathway Plan, Pathways Start and Guided Pathways, the client selects, contracts with and pays each outside provider directly. Pathways Complete and Pathways Custom may include Managed Provider Administration under a signed scope, including approved schedules, provider documents and authorized project payments. Unless a signed addendum expressly provides otherwise, the provider still contracts directly with the client and remains responsible for its work. Long Pathways receives no referral fees.
