In a client record, an authorized participant may scan an assigned label to open the current item details. QR use depends on the selected Pathway and is never required.

Outside providers are independently selected and contracted by the client. Complete and Custom may include Managed Provider Administration, but final provider selection and consequential spending approvals remain with the client or authorized representative. Tools and coordinator involvement vary by the selected Pathway and approved scope.

When Managed Provider Administration is included in the signed scope, the Portal can keep provider options, approvals, schedules, authorized expenses, invoices and completion evidence connected to the household plan.

There is currently no separate household Portal subscription fee. Long Pathways may extend Early Access while the experience is refined. Any future paid option will require advance notice and your affirmative enrollment.

A limited number of households may use the self-guided Family Portal while helping Long Pathways refine it. Early Access includes family collaboration, belongings, decisions, dates, messages, documents and export—but not coordinator case management, provider coordination or hands-on transition services.
