GUIDANCE FORPlanning for myself

This guidance is organized around your preferences and the level of involvement you want.

Change perspective

All Long Pathways services remain available. This perspective changes the language, examples and order—not the services you may consider.

PARTICIPATION AND APPROVAL

You decide how involved you want to be.

Having decision-making authority does not mean you must personally manage every detail. Select the description that comes closest to your situation. You can change it later.

Choose the description closest to your situation

Select the description closest to your situation to personalize what you see next.

I’m not ready to choose—show me all services

Long Pathways records the client, decision-maker, main coordinator contact, participation preference and approved viewers separately. It does not determine legal capacity.

BEGIN WITH YOUR SITUATION

Begin with what you want life at home—or your next home—to feel like.

You can prepare gradually, focus on one room or decision at a time and choose only the coordination you want. A move is not required to begin.

Stay at home with a plan

Organize the household, future intentions and approved support around your priorities.

EVERYTHING LONG PATHWAYS CAN HELP ORGANIZE

All services, explained through your situation.

Your perspective changes what is emphasized first. It does not hide or remove services.

Services are selected and scoped according to the household’s needs. Not every service is included in every Pathway, and outside-provider charges remain separate unless expressly included in a signed scope. See the complete service framework →

THE COMPLETE RANGE OF SUPPORT

Your perspective changes the guidance—not the services available.

Compare the full support ladder and the related services that may become relevant as the situation becomes clearer.

ONE DEFINED METHOD

From the first conversation to documented completion.

Your advisor first documents what matters to you, then turns approved decisions into a visible plan. Coordination begins only after you approve the scope and next step.

  1. 01

    Assess

Clarify the situation, goals, authority, risks and timing.

  2. 02

    Document

Create the shared record for facts, belongings and open questions.

  3. 03

    Decide

Record the client’s or authorized person’s approved choices.

  4. 04

    Coordinate

Connect responsibilities, dates, providers and supporting records.

  5. 05

    Complete

Verify outcomes and release a dated completion record.

FOR EACH MEANINGFUL BELONGING

Every item can move from “needs a decision” to a visible next step.

Long Pathways photographs and identifies belongings as permitted by the selected scope, records the approved outcome and keeps responsibility and current status connected.

ONE SHARED FAMILY RECORD

Invite trusted people without making technology another burden.

The Family Portal can show approved belongings, responsibilities, dates, messages and documents to the people you choose. Decisions may also be gathered by phone, video or in person and documented by the coordinator.

  • Approved decisions and current status
  • Responsible people, dates and next actions
  • Messages, documents and completion evidence

OUTSIDE-PROVIDER OPTIONS

We organize the proposal. You make the final selection.

When outside help is needed, Long Pathways identifies qualified options, organizes comparable information and presents a proposal. You—or another properly authorized person—make the final selection and contract with the provider directly.

  1. 01Confirm the need

Document scope, authority, timing and selection criteria.

  2. 02Organize options

Gather qualified providers and comparable information.

  3. 03You select

The client or authorized person gives final approval.

  4. 04Coordinate and record

Keep approved work, dates and completion records visible.

Long Pathways coordinates the process; it does not make the client’s decision. Independent providers contract under their own responsibility, and their costs remain separate unless an approved client-funded project reserve is expressly included in a signed scope.

CHOOSE THE AMOUNT OF COORDINATION YOU NEED

Understand the options before making a commitment.

Start privately, preview the deliverable or talk with an advisor. The $495 Transition Assessment & Pathway Plan can stand alone; continuing with Long Pathways is your choice.

A FAMILY-OWNED COORDINATION COMPANY

Preparation should support independence—not take it away.

Long Pathways is a family-owned coordination company. Your advisor organizes the work, documents decisions and keeps the next responsible action visible while your priorities remain central.
