Transition Assessment & Pathway Plan™

A complete standalone service that turns your circumstances, priorities and deadlines into a personalized 30-day action checklist, a coordinator-reviewed 30/60/90-day written plan, a plan presentation and six months of Family Portal access.
A focused 5-hour onsite session that uses your plan to complete one belongings, decisions or household-organization priority, followed by an organized handoff.
Your family performs much of the physical work while Long Pathways establishes the shared plan, prepares decisions and maintains momentum through scheduled coordinator checkpoints.
Your coordinator is present for the agreed decision sessions, move day and settling in, and keeps the providers you select, the Portal and the transition details connected through closeout.

An individually designed engagement for larger estates, multiple homes, longer planning ramps, aging in place, substantial collections or household work that does not fit a standard Pathway.
If the same household later enters a signed Pathways Start, Guided Pathways, Pathways Complete or Pathways Custom engagement, the full $495 planning payment is applied once as a credit against the Long Pathways service fee shown on that engagement's invoice. The credit does not apply to third-party provider charges, travel, reimbursable expenses or project reserves; it is nontransferable and has no cash value.
