Use your Transition Assessment & Pathway Plan independently, share it with people you authorize, or separately choose another Long Pathways Pathway. The service includes a personalized 30-day action checklist, a coordinator-reviewed 30/60/90-day roadmap, a plan presentation, a seven-day clarification period and six months of Portal access. If the same household later enters a signed larger Pathway, the full $495 is applied once against the Long Pathways service fee shown on that engagement's invoice.
